Fundamentals: Contemporary guided surgery systems are being incorporated into everyday clinical practice; nevertheless, its efficiency and accuracy is yet limited. Using a meta-analysis, this review is aimed at assessing the available evidence in the literature about the accuracy of oral implantology guided surgery systems. Material and Methods: PubMed engine was used to carry out a systematic search for English articles published no later than September 2009 with information about the accuracy of guided surgery systems. The mean average (Δ) obtained at the drill or implant entry points was analyzed following a random effects model. Subgroups were established and analyzed based on the system used, type of measured area or study design. Results: In the 20 studies selected, the mean deviation at the drill entry point was 0.73 mm. (IC 95%: 0.59-0.86), 3.4 mm maximum. Conclusions: Differences in the designs of the studies could not make it possible to identify a system that was superior to the rest. Further clinical studies are required, especially regarding navigation systems.
