Introduction: Neonatal studies quantifying medication errors are scarce. Differences in terminology used have complicated the comparisons. Objective: To quantify de incidence of medication errors at the neonatal unit during prescription and transcription. To identify variables affecting the incidence of medication errors. Method: This is an observational prospective study conducted in medium an intensive care of neonatal unit. Ruiz-Jarabo classification was used to identify medication errors observed during prescription and transcription. Descriptive and binary logistic regression analyses were used. Results: In the prescription phase 18 medication errors were detected per 100 prescriptions and during transcription phase 41 medication errors were detected per 100 transcriptions. Prescription errors were related to experience of prescriptor, dosage form, route and frequency of administration, number of prescriptions and patient characteristics (the need of parenteral nutrition, central venous access, sympathomimetic amines and ventilatory support). Transcription errors were related to day shifts, days, frequency of administration and number of transcriptions.
